SMS (“Texts”) Terms and Conditions
When you sign up for text messages from Ballad Health and its Affiliated Covered Entities, you are signing up to receive text messages related to your relationship with Ballad Health and Affiliated Covered Entities, including updates related to your visits, MyChart account, one-time passcode, billing notifications, prescription reminders, and care management.
How to opt in
To opt-in to text messages, text JOIN to 47649.
How to opt out
You can opt-out of SMS messages by texting STOP to respective short code. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the short code you opted out from. If you want to join again, sign up using Ballad Health MyChart or text HELP to the short code for instructions.
Please note that by signing up for text messages, you acknowledge that messages from the Ballad Health text program, which may include some limited protected health information, will be sent to the mobile phone number you provide, which may be viewed by whomever has access to that mobile phone or the carrier account. Messages sent as part of the Ballad Health text program are unencrypted. Unencrypted messages may be intercepted or received by unintended third parties, and/or stored or archived by service providers and system operators.

Carriers are not liable for delayed or undelivered messages.
Message and data rates may apply for any messages sent to you from us and to us from you. Message frequency may vary.
